These clients are enrolled in the Community Services Program (CSP).This position will team with the program social workers/case managers within the CSP program to assist in community integration for clients. The ability to be a consistent presence in clients' lives helps provide stability and better outcomes for the client's ability to be integrated and remain successful within the community. This position will require evening and weekend hours.This position works under the direct supervision of the Behavioral Health Manager of Wraparound Treatment Services. January 1, 2025 the new wage will be $20.34/hour - $23.93/hour Essential ResponsibilitiesAssist clients in successfully managing, home care, medication checks, and/or daily living assistance as needed. Manage client's medication schedule based on client/employee location.Transport and support clients to appointments or other community integration and daily living skill activities. Produce case notes providing activities performed and observations of the client's behaviors to provide feedback to the program case managers, ensuring clients' wellbeing.Monitor client symptoms and document progress and obstacles. Support DHS vision of helping the people of Waupaca County to be Safe and Connected.QualificationsEDUCATION: High school diploma or equivalent. EXPERIENCE: Current and/or past experience working with individuals with a mental health or AODA (alcohol and other drugs addiction) diagnosis. SPECIAL REQUIREMENTS: CSP (Community Services Program) initial training of 8 hours with ongoing supervision.
