Collaborate with healthcare providers, and community organizations to enhance the delivery of health services and promote health outcomes Assistance including: meeting with members in their homes, nursing homes, shelters, or doctor's offices, etc.Required QualificationsMinimum 1 year experience working with underserved or special needs populations, with varied health, economic and educational circumstances Bilingual with Spanish and Bengali High school diploma, preferably a Bachelor or Associate degree
