- Under General supervision of Ancillary Services, will provide direct client assessments and counseling to the population of the homeless individuals and families who are affected by Mental illness and/or co-occurring services in the WCMH Homeless/Housing Program.
- Provides outreach services to homeless clients with serious mental illness and co-occurring disorders.
- Will assists clients in obtaining and maintain safe, decent and affordable housing. Enters contact data into Homeless Management Information System (HMIS) of South Carolina.
- Provides general mental health, substance abuse and psychosocial assessments in identified local shelters, emergency rooms, probate court and criminal justice systems, A&D Commision, and other identified sites.
- Acts as a liaison between the homeless/housing program and other helping agencies. Assists in facilitating the purchase of supplies for homeless clients. Attends local mental health center staff meetings Attends individual and group supervision meetings.
- Maintains documentation of charts and other records in accordance with the Federal, State, Local, SCDMH, WCMH, QA, Corporate Compliance and CARF standards.
- Ability to establish and maintain working relationships with client, families and representatives of other professional disciplines.
- Ability to communicate effectively. Employee will work in the Conway Clinic in the Housing/Homeless Program.
- Traveling, Lifting and standing required.
- Perform other related duties as required by Supervisor and Executive Director.
