With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina. This position is located at the Aiken-Barnwell Mental Health Center, 1135 Gregg Highway, Aiken, SC 29803. This position will involve delivering services in the community and in patients' homes. Job Purpose Serves on an Assertive Community Treatment (ACT) team providing evidenced based services in an intensive non-residential treatment and rehabilitative services in accordance with the fidelity model. This is a single fixed point of responsibility for treatment, rehabilitation, and support needs for patients with Serious Mental Illness (SMI) who require higher levels of community care and have not been well supported in lower level of care options. Job Functions: Provides direct services to ACT patients guiding them in the development of skills/strategics towards achieving and maintaining good mental health and facilitating training of independent living skills. Utilizes assertive engagement to engage patients in service. Assists patients with access to community services, resources and agencies and establishing support networks in the community. Completes screenings for referrals and assists with development of treatment plans. Provides family psychoeducation and works with patient's natural supports. Participates in clinical supervision, treatment team meetings, pursues professional development, and demonstrates effective use of social and interpersonal knowledge and skills and demonstrate an understanding and appreciation of individual and group differences and similarities. Attends treatment team meetings daily and provides consultation regarding complex cases as appropriate. Completes documentation according to QA/SCDMH standards, Complies with Corporate Compliance Policy and Risk Management Plan, and Ensures timely completion of prior authorizations. Completes all annual required trainings to include Safety, MAPS, Cultural Affairs, and clinical trainings. Demonstrates adherence to CARF standards, and SCDMH and Center policies and Procedures. Performs other duties as needed State Minimum Requirements: A bachelor's degree in guidance, counseling or the social sciences and professional experience in human services or social services programs. A gency (Facility) Minimum Requirements: At least one years' experience working with adults with SMI. Note: If any Agency Requirements are listed above, applicants must also meet those requirements to be considered for the position. Agency (Facility) Preferences: Must qualify to render services applicable to position per credentialing and privileging standards and with concurrence by DMH Office of Quality Management. Considerable knowledge of the techniques of interviewing, counseling and guidance. Ability to establish rapport with the patients. Ability to communicate effectively. Supervision provided by ACT team leader and Psychiatric care provider. Employee's activities are governed by Center and SCDMH policy and procedure. Ability to lift 5lbs and operate a state vehicle. Position is considered essential and may be called back during crises/disasters/emergencies. Position will be part of the Assertive Community Treatment Team. Requires flexible schedule. Services are delivered in the community and patients' homes. Bilingual abilities in English and Spanish (or another language) are a plus.
