Credentialed Chemical Dependency Counselor, UHS New Horizons. Shift. Day. Hours per week: 40 Salary range: $25.27 - $37.90 per hour, depending on experience. Overview. Manages a caseload of patients and completes assessments, problem lists, treatment plans, treatment plan reviews, progress notes, discharge plans and discharge summaries. Actively participates in up to four times a week staffing's, bi-weekly supervision, completed treatment plans and treatment plan reviews for staff discussion and approval. Education/ Experience. Minimum Required:Associate degree in applied science in Chemical Dependency or related field; OR two (2) years experience working in a chemical dependency setting.?License/ Certification.
