The full continuum of River Edge Behavioral Health services provides state-of-the-art treatment and support services for more than 13,000 children, youth, families and adults annually, who experience mental illness, addiction, co-occurring disorders or developmental disabilities.In this role you will........Facilitate and coordinate the reintegration of clients from an inpatient environment back to the community.Job SummaryRiver Edge is looking for experienced Hospital Discharge Liaison or recent Master's degree graduates in the field of Social Work, Psychology, Counseling or behavioral health related fields.Location/Schedule:Schedule: Monday- Friday 8:30AM-5PMFLSA Classification: HourlyBenefits Offered:* Competitive Pay* Medical, Dental, Vision Insurance* Paid Time Off* 12 Paid Holidays* 401k with an employer match* Flexible Spending Accounts* Short and Long-term disability coverage* Life Insurance* Employee Discount ProgramDuties & Responsibilities:* Duties include building and maintaining relationships with discharge authorities, facilitate discharge by coordinating all appropriate procedures working in partnership with interdisciplinary treatment team, other agencies, ensuring that all plans are in place for the time of discharge to ensure continuity of care.Qualifications.* Masters degree in Social Work, Sociology, Criminal Justice, and/or Psychology.* Candidates eligible for licensure as LCSW, LMFT, LPC/LMSW, LAMFT, LAPC are preferred.* Strong problem-solving and critical thinking abilities to assess patients' needs, identify potential barriers, and develop effective discharge plan.
