* Job Title: Supervisor - Designated Coordinator

* Location: Brainerd, MN (Serving Crow Wing, Morrison, Aitkin, and Northern Mille Lacs Counties)

* Employment Type: Full-time, Exempt, Grant funded through June 2026

ABOUT THE ROLE

We are seeking a motivated and experienced Supervisor to lead the development and growth of unit based services in the Brainerd area. This position, funded through the HCBS Provider Capacity Grant from the Minnesota Department of Human Services through June 2026. This position offers a unique opportunity to transform lives in the community. You will play a pivotal role in building unit-based services, fostering community connections, and supporting the individuals we serve.

KEY RESPONSIBILITIES:

* Service Growth: Lead the expansion of Individualized Home Supports and 24-Hour Emergency Assistance services.

* Community Engagement: Collaborate with local agencies and stakeholders to identify community needs and expand offerings; perform outreach across the service area.

* Staff Development: Provide training, oversight, and mentorship to ensure staff compliance with state and federal regulations.

* Operational Excellence: Monitor service outcomes and implement continuous quality improvement measures; support and manage staff schedules, caseloads, and professional development.

* Leadership Collaboration: Communicate and collaborate closely with the Area Director to align organizational goals.

QUALIFICATIONS:

* Education: Bachelor's degree in Human Services, Social Work, or a related field preferred.

* Experience: At least 4 years of supervisory experience in a similar setting, including working with individuals with disabilities.

* Compliance Knowledge: Expertise in Minnesota Statutes 245D.081, Subd. 3, including:

* Maintaining a current understanding of licensing requirements.

* Ensuring duties of the designated coordinator are fulfilled.

* Implementing corrective actions for incidents and emergencies.

* Evaluating satisfaction with service delivery and progress toward outcomes.

* Ensuring staff meet competency, orientation, and training requirements.

* Taking corrective actions ordered by the commissioner.

* Documenting and implementing program improvements.

* Additional Requirement: At least 3 years of supervisory experience in programs serving vulnerable adults or children.

SKILLS AND ATTRIBUTES

* Exceptional communication and organizational skills.

* Strong conflict resolution and problem-solving abilities.

* In-depth knowledge of 245D regulations and licensing standards.

* Proficiency in industry-related software (e.g., Therap, ADP).

* Ability to manage multiple priorities and maintain professionalism under pressure.

* Commitment to fostering a cooperative and inclusive workplace culture.

ADDITIONAL REQUIREMENTS

* Valid driver's license, current auto insurance, and reliable transportation.

* Flexibility to work occasional evenings or weekends as needed.

* Frequent travel between clients required.
