Identifies patients at nutrition risk and assesses nutrient requirements according to age and disease specific needs. Develops a Nutrition Diagnosis and implements nutrition care treatment plans for all assigned patients. Continuously evaluates and monitors the effectiveness and outcomes of MNT intervention.Responsibilities:EDUCATIONBS Degree in Nutrition/Dietetics plus internship or other education to meet requirements for registration with the Commission on Dietetic Registration (CDR)Registration with the CDRRegistered Dietitian or registration eligible. Registration must be obtained within 6 months of employmentCertified Dietitian-Nutritionist (CDN) with New York State to obtained within 1st year of employmentActive membership with the Academy of Nutrition and Dietetics preferredEXPERIENCE1 year experience preferredKNOWLEDGE, SKILL AND ABILITYMust possess or have ability to learn basic computer skillsWORKING CONDITIONSOccasional exposure to one or more mildly unpleasant physical conditionsTravel between Catholic Health System facilities may be requiredOccasional evenings may be required for nutrition counseling appointments or classesREQNUMBER: 31792
