Sigma HomeCare is a leading provider of home care services for adults with severe mental illness (SMI), serious emotional disturbance (SED), intellectual and developmental disabilities (I/DD), and those in need of mentorship programs. We are currently seeking compassionate and dedicated Direct Care Workers to join our team and provide exceptional care to our clients.
As a Direct Care Worker at Sigma HomeCare, you will play a vital role in improving the quality of life for our clients. You will be responsible for providing comprehensive community support services and respite, ensuring that our clients receive the highest standard of care in the comfort of their homes.
Responsibilities:
- Direct Care and Support: Provide direct care and support to clients in their homes, assisting with daily living activities such as bathing, dressing, grooming, and meal preparation. Ensure clients' comfort and well-being at all times.
- Medication Monitoring: Monitor clients' medications as prescribed by healthcare professionals. Observe for any side effects or adverse reactions and report any concerns to the appropriate healthcare provider.
- Individualized Care Plans: Implement and follow individualized care plans tailored to each client's specific needs. Regularly review and update care plans in collaboration with healthcare professionals and family members.
- Monitoring and Documentation: Monitor clients' progress and behavior, keeping detailed records of their daily activities, health status, and any significant changes. Maintain accurate and up-to-date documentation for each client.
- Communication: Communicate effectively with clients, their families, and other healthcare professionals to ensure coordinated and comprehensive care. Provide updates on clients' progress and address any concerns or questions.
- Transportation: Transport clients to appointments, community activities, and other necessary outings. Ensure clients' safety and comfort during transportation.
- Environment Maintenance: Maintain a clean and safe environment for clients. Assist with light housekeeping duties, including laundry, cleaning, and organizing clients' living spaces.
Requirements:
- High school diploma or equivalent.
- Strong communication and interpersonal skills, with the ability to build positive relationships with clients and their families.
- Ability to work independently and as part of a team, demonstrating reliability and accountability.
- Valid driver's license and reliable transportation are required.
- Must pass a background check and drug screening.
- Must have a smartphone to access work-related applications and communicate with the team.
