TITLE: Direct Support Professional, Day ServicesImmediate Supervisor: Program Director-Day ServicesSupervision Exercised: NoneSUMMARY: Under the supervision of the Program Director of Day Services, this position is responsible for assisting adults with developmental disabilities in promoting their independence in social and employment settings.ESSENTIAL FUNCTIONS:
- Provides on-the-job training to program participants in groups and/or individually
- Responsible for providing skill or developmental training in various activities as designated; assists with personal care (including toileting/changing)/hygiene
- Maintains accurate and current records such as daily participant documentation
- Contributes to positive relations between community members, program participants, vendors, and other Agency staff
- Trains program participants in accessing public transportation and assists them with community integration
- Monitors, tracks, and assists program participants in goal achievement
- Organizes social and vocational groups in promoting independence
- Complies with all Momentum Inc. program policies, CARF accreditation standards, TCRC, CAL/OSHA policies and procedures
- Must be able to work at other Momentum WORK, Inc. locations, when needed
- Must be able to transport participants, unless waived by the Executive Director
- Other related duties and responsibilities on an as needed basis
PHYSICAL REQUIREMENTS:
- Ability to stand, sit for long periods of time
- Ability to go up and down stairs
- Ability to walk for long periods of time
- Ability to push or pull wheelchairs when necessary
- Ability to speak, read, hear, and understand technical written materials
- The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served
- Full awareness of environmental stimuli
- Ability to implement Crisis Management, Safety & ProAct techniques (training included)

- High school diploma or equivalent
- At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred
- Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information
- Excellent verbal and written communication skills
- Ability to complete tasks with numerous interruptions
- Compassion and respect for persons with disabilities
- Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations
- May need to be available during any disasters, per disaster plan
- Must have reliable phone
- Valid CA Driver's License: must have an acceptable driving record as determined by criteria established by the Agency's insurance carrier and by Agency policy
