Works as part of an interdisciplinary team. Assists in the development, delivery and coordination and monitoring of skill training and recreation planning with residents and staff in keeping with program plan, schedule and work assignment. Provides supportive direction and hand-on guidance in life Skills Training to residents of the Community Residence. Assists with creating an environment for the development and delivery of diverse social skills training and utilizing both in-house and community based resources. Assists residents with planning, purchasing and preparing meals. Assists with functions required to run a home including but not limited to housekeeping, facility maintenance and laundry. Completes all documentation and record keeping as outlined by unit guidelines and regulatory body requirements Ensures all documentation is objective. Reports resident behavior or events and outcomes in factual terms in progress notes, communication books and Medicaid billing log. Progress notes are to be completed bi-weekly and a monthly restorative service note. All progress will summarize the given time frames, events, resident progress, outside liaisons and any other pertinent information. All documentation is legible and in black ink in charts and forms. Completes all documentation to ensure accurate medical billing information which reflects the individual service plan. Document must include identification of service delivered, duration of service, delivery and outcome of the intervention. Documentation may include Medicaid log book, progress notes and monthly restorative service note. Provides input to behavioral specialist to assist in development implementation and evaluation of individual service plans. Possesses the skills to deliver effective person-centered care with respect to the ongoing symptomatology and recognize individual resident's challenges. Ensures access or referral to identified psychiatric, medical and dental needs of the individual. Demonstrates ability to identify changes in resident behavior, mood and affect and reports findings appropriately and takes necessary actions as directed or indicated. Has a working knowledge of psychotropic medications and supervisions of self-medication of residents as per policy. Know signs and symptoms of side effects of medications. Demonstrates ability to assess and identify signs and symptoms of alcohol or drug intoxication and intervenes when needed. Ensures accuracy of self-medication documentation and controlled substance counts Remains flexible and available as it relates to personally providing coverage for a variety of emergencies, holiday, relief and weekend shift coverage, as needed. Qualifications: High School Diploma/GED required. Bachelor's degree preferred. Experience with a special need or mentally disabled adults preferred. Ability to write legibly required. Maintain valid NYS driver's license within agency criteria. Salary Range USD $21.00 - USD $24.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
