Qualifications include:
- Masters in Human Services required (LMSW preferred)
- Minimum of 5 years' experience in OPWDD admissions and procedures
- Knowledge of OPWDD services and entitlements
- Strong ability to publically speak and communicate to families and external providers
- Excellent writing skills
- Valid New York State driver's license
Responsibilities will include:
- Supervise the Assistant Director of Admissions and ensure comprehensive file management for all admissions and discharges
- Facilitate the admissions process for all Life's WORC OPWDD programs in a timely manner
- Lead the agency's Admissions and Discharge Meetings, incorporating all department administrators in key programmatic decisions that pertain to admissions and discharges
- Schedule and participate in all internal placement review meetings, screenings, etc.
- Coordinate new development activities as assigned
- Participate in New Development planning meetings
- Coordinate family pre-admission meetings (i.e. guest speakers, presentation materials, etc.) for all new developments, including periodic development updates
- Participate in all required OPWDD referral/development meetings
- Create essential data systems and recording tools needed to ensure efficiency of the department
- Record all demographic information in Medisked
- Distribute all pertinent admissions data identifying all Key Performance Indicators, including monthly Vacancy Report, Loss Revenue Report, acceptance rate, etc. to the Chief Officer of Residential and Clinical Operations and Vice President of Admissions and Therapeutic Services monthly
- Ensure distribution and collection of all Admissions documents (i.e. consents, DDP2s, LOCs, NODs, etc.)
- Submit required documentation to OPWDD monthly
- Provide outreach and coordination with OPWDD, families, and the community (i.e. attend resource fairs, transition fairs, present informational sessions, etc.) for Life's WORC to enhance referrals and provide educational information to the community
- Assume the role of OPWDD liaison, maintaining consistent contact and collaborative efforts
- Maintain ongoing contact and communication with all school-based transition support liaisons
