Pay: $150,000.00-$160,000.00

This Hospice Director of Case Management/RN will be responsible for the following duties:

• Oversee the daily operation of the Case Management Department.
• Coordinate hospital wide functions of Case Management, Utilization Review and Discharge Planning.
• Oversee and coordinate the collection of pertinent Hospital data.

REQUIREMENTS:

• Current NYS unencumbered RN license
• Minimum (5) years in Nursing/Case Management/UR or Discharge Planning, Two years in supervisory role.br> - Case Management certification required within 2 years of employment.
