Together.The Director of Social Services is the lead for all clinical and administrative requirements of the as-signed programs to include activities such as plans to serve the highest risk members. Partners with clinical and executive leadership on all aspects of the assigned Social Work and Health Coach pro-grams to include developing and implementing a strategy, key initiatives, and associated action plans, and key performance indicators (KPIs) to track program performance. Provides departmental direction and day-to-day operational oversight to the Social Work and Health Coach teams supporting Alignment members. Oversees the development and implementation of policies and procedures that support the organizational and clinical goals and is responsible for monitoring health outcomes and trends.
Job Duties/Responsibilities:1. Oversees the process, management and tracking of program referrals and enrolled members. 2. Provides guidance to staff or directly manage complicated Social Work requests from members, providers or staff.3. Assists with the development and oversight of Social Work and Health Educator programs, pro-jects, workflow, standard operating procedures and reports.4. Creates/Reviews workflows, job aid, standard operating procedures, and/or policies and rec-ommend or change as appropriate to ensure timely, efficient, and effective outputs.5. Collaborates with other supervisors and managers in the department to improve processes and workflow.6. Assist with assessment activities and other process improvement strategies to enhance the ef-fectiveness of the Social Services department.7. Acts as a resource to the members, providers, and community partners.8. Builds team engagement and efficiency to drive improved member and program outcomes. 9. Communicates with internal and external customers in a professional manner and follows through on commitments. 10. Designs and implements programs to address social services needs within the Alignment popu-lation. Initiate and follow through on projects and deliverables. Track program progress and outcomes and present key findings to clinical leadership. 11. Provides supervision to MSW level team members working towards their LCSW. Maintain/obtain qualifications to support this function. 12. Other projects and duties as assigned.
Supervisory Requirements:Fulfill supervisory responsibilities in accordance with organization policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Mentors, trains, audits, coaches and manages team of Social Workers and Health Educators to ensure compliance with Medical Management policies and procedures and ensure compliance with all regulatory requirements.
Job Requirements:Experience:Minimum Experience:•Required: Minimum 5 years Social Work experience with a minimum 2 years of experience as a LCSW.•Preferred: 5+ years supervisory/lead experience in a health care setting. LCSW license training supervision experience.
Education:•Required: Master's Degree in Social Work.
