* JOB REQUIREMENTS* Education* Skills and proficiency in oral and written grammar, communication, and basic mathematics as acquired through official completion of high school or its equivalent. Ability to perform support activities in patient care as required through completion of an approved Vo-Tech program or certified as a Nurse Aide through the Office of Long Term Care, graduate of the SBMC PCT program, or after completion of an RN/LPN Nursing Student Skills Checklist completed by their nursing instructor. Candidates must be currently enrolled in an approved program to utilize the RN/LPN Nursing Student Skills Checklist. PCT experience within the last year will be considered in lieu of formal training.* Experience* Knowledge of job content, patient care practices and requirements as acquired by on-the-job training and knowledge of hospital and departmental policies and procedures as acquired through orientation and preceptorship. Health Care and clerical experience preferred.* Physical* This is a safety sensitive position. Please see the St. Bernards Substance Abuse Policy for further information.* Normal hospital environment. Continuous exposure to biological hazards. Frequent exposure to unpleasant fumes and odors. Close eye work. Hearing within normal and soft tones. Distinguish smells. Distinguish temperatures by touch and proximity. Lifting up to 100 lbs. Carrying up to 40 lbs. Pushing/pulling up to 350 lbs. Frequent sitting, standing, walking, bending, stooping and reaching.* Additional Skills* Excellent interpersonal and customer service skills. Working knowledge of personal computers and ability to learn a variety of software programs. Proficient spelling and grammatical skills. Attention to detail with excellent planning and organizational skills. Communicate effectively in both oral and written forms. Ability to operate standard office equipment including, but not limited to, a calculator, fax machine, copier, printer and scanner. Ability to successfully maintain effective work relationships. Ability to maintain organization and multitask.* JOB SUMMARY* The Discharge Coordinator/PCT serves as the facilitator of the patient throughput/discharge process and is a valued member of the healthcare team. They collaborate with case managers and other members of the healthcare team to implement the discharge plan agreed upon by the RN, case manager, physician, patient/chose support system, and other involved members of the interdisciplinary team. Responsible for performing assigned patient care activities and assisting licensed nursing personnel. The role is designed to support a wide variety of operational support functions that support discharge-planning activities.
