The Enhanced Care Management (ECM) Program, operating under the umbrella of Fred Finch CARES, is a comprehensive care coordination benefit launched in July 2023 under CalAIM (California Advancing and Innovating Medi-Cal). ECM adopts a Whole Person Care Approach to healthcare, providing physical, behavioral, emotional, and social services to enhance care coordination and health outcomes. It is a care-coordination service offered to children, youth, and their families insured through Managed Medi-Cal. With a cross-regional services delivery model in both Northern and Southern California, ECM aligns and coordinates services across multiple systems of health and behavioral healthcare while addressing social determinants. Families with children, teens, or young adults up to 21 years old, youth up to 26 years old if involved in the Foster Care System, children, teens, or young adults enrolled in CCS/CCS Whole Child Model, children, and families at risk of/or experiencing homelessness, and children with mental health or substance use conditions are eligible to enroll in the ECM program. The program offers a thorough assessment incorporating the Social Determinants of Health, tailored services to meet each family's unique needs, dedicated care manager for each enrolled family, and care coordination, outreach and engagement, health promotion, comprehensive transitional care, linking families to services, advocacy, and access to resources. Join our team as: Program Supervisor- ECM What You Will Do The Enhanced Care Management (ECM) CARES Program Supervisor ensures the quality of services provided by the ECM Services Unit that addresses the clinical and non- clinical circumstances of high- need clients enrolled in Medi- Cal managed care. The supervisor works under the direction of the Sr. Director - ECM CARES ECM services provide a whole-person approach to children, youth and families experiencing complex medical, behavioral and psychosocial challenges to minimize the likelihood of preventable hospital and emergency department admissions. Services include outreach and engagement, providing person-centered care coordination, developing care plans, and providing linkages to medical, psychiatric, social, educational and other services as needed. The Supervisor provides direct supervision for Care Managers - ECM and supports ECM Services Unit activities related to staff training, assessment, quality assurance and improvement, and evaluation of the services. Performance is reviewed on a continuous basis with specific goals and objectives identified throughout the year. Shifts Available Full Time - Hourly; Mon-Fri/8am-5pm Salary RangeIn accordance with California law, the expected salary range for this California position is between $33.03 and $42.15 per hour. The actual compensation will be determined based on experience and other factors permitted by law.   • A.A. Degree with six years of work experience or a B.A. Degree and four years of work experience or M.A. Degree and two years of work experience.
  • Work experience should be in care coordination and case management for target population, such as children and adults experiencing Mental Illness (SMI) and/or Substance Use Disorder (SUD) or children involved in or with a history of involvement in child welfare.
  • Ability to organize and prepare documentation in a timely manner.
  • Ability to interact with the public or other employees in a professional, respectful, and courteous manner, practice good public relations and display sound judgment and decision- making skills in stressful situations, communicate in a professional and effective manner with others, both orally and in writing information, clearly and concisely, comprehend and follow verbal and written instructions.
  • Knowledge and/or experience working with children, youth, and families from various socio-economic and cultural backgrounds.
  • Possess initiative, flexibility, and the ability to work under pressure and exercise independent judgment.
  • A valid Class C California Driver's License, access to personal automobile, and proof of auto insurance.
  • Physical requirements: Able to hear a child calling for help, able to see a child on the playground, able to sit for long periods of time, seeing to inspect the field and written documents; ability to communicate with staff, affiliates, clients and public; sitting for extended periods of time; and dexterity of hands and fingers to operate office equipment.
  • Demonstrated cultural responsiveness in working with diverse families and communities.
  • Must be able to respond quickly to emergencies, and to learn and implement ProACT (or other agency approved crisis de-escalation training).
  • Clean driving record: current CA Driver's License or ability to obtain within 10 days after hired.
  • Must have access to dependable transportation; personal car insurance meeting minimum liability coverage required if position requires driving for work or transporting participants.
  • Basic computer and keyboarding competence with the ability to use Microsoft Office Suite and/or competency with electronic clinical documentation systems.
  • Preferred
  • Multilingual capabilities.
  • Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant or similar role.
  • Fluency in prevailing language(s) of the community.
