Job Type Full-timeDescriptionPOSITION SUMMARYProven experience in risk screening, comprehensive baseline evaluation or assessment, ongoing assessment and treatment (self-care training, nutrition education, counseling and support, and various interventions), and health outcome monitoring and evaluation. Provides counseling and nutrition education and counseling to pediatric and adult patients of The Wright Center patient centered medical home. In addition, the position will participate in clinic meetings and provide appropriate care under the direction of the clinical staff. Work is performed under general direction of the clinic medical staff and is evaluated through periodic conferences and the observation of program results. This position will also act as a resource for TWCGME's Sponsoring Institution as well as programmatic nutritional didactics and learning sessions.REPORTING RELATIONSHIPSThe position reports to the Clinical Chief Operating Officer for patient care duties and to the VP and Associate Designated Institutional Official for GME related duties. No employees report to this position.ESSENTIAL JOB DUTIES AND FUNCTIONSWhile living and demonstrating our Core Values, the Clinical Registered Dietician-Nutritionist & Diabetes Educator will:Assess and follow up on patient nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans and provide nutritional counselingConsult with physicians and health care personnel to determine nutritional needs and diet restrictions of patient or clientAdvise patients and their families on nutritional principles, dietary plans and diet modifications, and food selection and preparationConfer with physicians and other healthcare professionals to coordinate medical and nutritional needsFulfill both a therapeutic and preventative roleProvide nutritional counseling on an individual basis upon referral by clinicians as a part of the clinical treatment planProvide nutritional counseling for groups of patients needing ongoing assistance with management of a specific nutritional problem, basic rules of good nutrition, healthy eating habits, adherence and nutrition monitoring to improve their quality of life as a part of the overall clinical treatment plan Responsible for providing diabetes nutrition education activities for individuals and groups of patients with diabetes ranging in age from adolescence to adulthood Develop and led Sponsoring Institution and Programmatic didactics and learning sessions for residents and fellows. This included but not limited to: Fundamentals of nutrition and the diabetic continuum; Fundamentals of nutrition and GI disorders such as Spru and IBD; Fundamentals of nutrition and Cardiac disorders; Fundamentals of nutrition and longevity i.e. Lifestyle MedicineServes as a resource to other health professionals in the specified area of expertise for purposes of staff development, patient care delivery and Quality ImprovementParticipate in learning collaborative such as Weight-Inclusive Wellness in Community Health Centers: Bridging System and Care Coordination and any other program that TWC might pursue in the futureAssist in the development of nutritional curriculum in the organization's efforts to pursue other educational programsDevelop and maintain print and non-print educational materials on nutritional and diabetic topics for use by patients and cliniciansProviding in-service educational programs for clinicians on nutritional topics as neededRepresents and promotes The Wright Center at conferences, community events and community programs as neededMaintains and promotes patient confidentialityFollows all Medicare, Medicaid and private insurance regulations and requirementsActively participate in Quality Improvement InitiativesMaintains schedule flexibility to ensure availability to meet the needs of operational hours Thirty-six (36) hours dedicated to general nutrition inclusive of adults and pediatric patients under the direction of the clinic medical directors and Chief Medical OfficersFour (4) hours dedicated to nutritional didactics and educational programs in GME and learning collaboratives etc.Perform other duties as assigned RequirementsREQUIRED QUALIFICATIONSBachelor's Degree with a focus on nutrition requiredCurrent state licensure as a Registered DieticianCertified Diabetes Educator (CDE)Current BLS Certificate required Preferred minimum two years experiences in diabetes education in ambulatory or acute care settingExperience with electronic health record, Medent experience a plusStrong verbal and written communication skillsStrong computer skills to include MS Office SuiteKnowledge of a wide range of disease entities, treatment modalities and drug/nutrient interactionsThe ability to work well with physicians, nursing and other clinical team membersMeet The Wright Center for Community Health and its affiliated Enterprise entities' EOS People Analyzer ToolBuy in and experience working in the EOSĀ® model (strongly preferred)
