Location: Chicago, ILHospital: RUSH University Medical CenterDepartment: Breast Cancer TaskforceWork Type: Full Time (Total FTE between 0.9 and 1.0)Shift: Shift 1 Work Schedule: 8 Hr (8:00:00 AM - 5:00:00 PM)Summary:Equal Hope is a non-profit sitting on the Rush University Medical Center Campus. Equal Hope is dedicated to eradicating women's health racial, ethnic and class disparities and is a mission-oriented healthcare justice organization. Equal Hope is looking for a compassionate and dedicated Community Nurse Navigator to join our team and support women in need for a diagnostic follow-up after their screening mammogram and women diagnosed with breast cancer in navigating the healthcare system. The Community Nurse Navigator will work closely with safety net hospitals to provide comprehensive case management services to Equal Hope clients, ensuring they receive timely and effective care while addressing the social determinants of health.Other information:Required Job Qualifications:• Registered Nurse (RN) license in the state of Illinois. • Bachelor of Science in Nursing (BSN) preferred. • Experience in oncology nursing, case management, or patient navigation. • Knowledge of breast cancer diagnosis, treatment, and support services. • Strong interpersonal and communication skills. • Cultural competence and sensitivity to diverse populations. • Ability to work independently and collaborate with a multidisciplinary team. • Proficient in medical documentation and electronic health records. • Commitment to patient advocacy, empowerment, and holistic care. • Excellent written and verbal communication skills. • Passion for social justice, equity, and advocating for underserved communities. Preferred Job Qualifications:• 5 - 10 years of nursing experienceResponsibilities:Job Responsibilities:• Oversee the case management of Equal Hope clients who need a diagnostic follow up and/or those diagnosed with breast cancer. • Assess clients for social determinants of health impacting their care. • Coordinate and schedule medical appointments for clients. • Advocate for clients' healthcare needs and preferences within the healthcare system. • Accompany and navigate clients through appointments and healthcare processes. • Facilitate access to financial support and programs for eligible clients. • Connect clients with community resources and support services. • Provide culturally sensitive and competent care to diverse populations. • Educate clients on their diagnosis, treatment options, and self-care practices. • Collaborate with healthcare providers, social workers, and other stakeholders to ensure coordinated care. • Improve clients' health literacy and understanding of their healthcare journey. • Empower clients to actively participate in their treatment and decision-making. • Conduct follow-up with clients to monitor progress and address any concerns.• Maintain accurate and up-to-date case records and documentation. • Facilitate or connect clients with breast cancer support groups and resources. • Provide emotional support and coping strategies to clients and their families. • Participate in quality improvement initiatives to enhance client care outcomes. • Stay informed about best practices and advancements in oncology and case management.Rush is an equal opportunity employer.
