Job DescriptionSchedule:Mon - Fri 8am - 4pm or 4 days 8am - 6pm;This position will also require flexibility based on program needs and weekend hours on a rotational basisSalary:$87,000 / yearSign On Bonus:$5000; $1666 upon completion of 30 days, $1667 upon completion of 6 months, $1667 upon completion of 1 yearWe are seeking a new PACT Nurse for our Program for Assertive Treatment (PACT) teams to provide treatment to adults with persistent mental illness. The PACT Nurse will work under the supervision of the Lead Nurse to conduct psychiatric assessments in addition to physical health needs in order to provide holistic nursing services. The PACT Nurse will be responsible for making appropriate referrals to community physicians, providing management and administration of medication in addition to providing a range of treatment, rehabilitation, and support services.Other responsibilities include but are not limited to:
  • Provide case management for an assigned group of persons served, including coordinating and monitoring the activities of the individual treatment team
  • Conduct a comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health
  • Provide ongoing assessment, problem solving, side-by-side services, skill training, supervision, and environmental adaptations to assist persons served with activities of daily living.

  • Two years experience in a mental health setting.
  • RN in state of service required.
  • A high school diploma or equivalent is required; Bachelor's degree preferred. In some cases, experience may be substituted for education.
  • Strong clinical and crisis intervention skills, competency in the use of formal and informal assessment tools and practices
  • Ability to work independently as well as a part of a multi-disciplinary team
  • Must be at least 21 years of age and have a current, valid, US drivers' license for at least one year. Additionally, this position requires a fully insured vehicle that you will use in the course of your daily work. You must be willing to transport clients in your vehicle.
  • Must be able to successfully pass a CORI, reference, multi-state background, professional licensure and driving record check.
  • Must be able to become CPR and First Aid certified within 2 weeks of employment and PACT Module Training within 3 months of hire
