Evaluation and Referral Clinical Coordinator - St Vincent Hospital : Job Details

Job Description The Evaluation and Referral Clinical Coordinator is a licensed individual, who is either a LMSW, LCSW or a LMHC and based on training and professional experience, can provide a comprehensive evaluation to patients seeking services. The coordinator can determine and refer to appropriate level of care. The coordinator can recognize patients who are experiencing acute psychiatric symptomology, which includes assessment of risk for suicide and addressing/reducing suicide risk. The coordinator is responsible for oversight of ambulatory intake and referral, this includes doing diagnostic and level of care assessments. The coordinator is also responsible for recruiting and overseeing other licensed therapist working within the Evaluation Referral service. The coordinator collaborates closely with the physicians on the team and routinely starts cases that the physicians may complete if the patient requires inpatient admission. The coordinator contributes to the tracking and reporting of data including trends and the maintenance of professional relationships with internal and external customers.
