The Facilitator is responsible for the overall coordination of intensive, family-centered, community-based services for approximately 8-11 families in the Wraparound program. This position facilitates Child and Family Team (CFT) Meetings and guides the evolution of the Individual Child and Family Plan in order to promote a successful home placement for the referred youth. The Facilitator focuses on accessing and building upon the family's natural strengths and community resources. This position may expected to be available after-hours or part of an on-call rotation. PROGRAM DESCRIPTION: The Success First program serves children & youth ages 0-21 who are at risk of loss of placement (e.g. children involved with the foster care or juvenile justice systems, those experiencing psychiatric hospitalizations, suspensions/expulsions, and eating disorders). Children are typically referred from hospitals, schools, CFS, probation, or Department of Behavioral Health. Services are provided in clients' homes, schools, or local communities. Children typically remain in the program for 3-6 months with the goal of reducing disruptions to their placements by improving their ability to successfully manage their mental health needs. JOB DUTIES & RESPONSIBILITIES:•Effectively manage and coordinate all aspects of the Child and Family Team (CFT).•Perform initial and ongoing strengths-based assessments of children and families. Identify and prioritize family's needs across all domains.•Schedule and facilitate all Child and Family Team (CFT) meetings; involve the family in the planning and decision-making process. •Role model positive communication and conflict resolution skills. •Identify opportunities and resources to promote family strengths, self-sufficiency, and independence. •Attend DBH and CFS office hours to support coordination of care with social workers and external support services.•Maintain proficient and accurate documentation adhering to all program and contract standards for compliance, productivity and timeliness. •Demonstrate leadership within the multidisciplinary Success First team and utilize a Team Concept to foster effective collaboration.•Interface and collaborate with Department of Children and Family Services, Department of Probation, Department of Behavioral Health, law enforcement, schools, health care providers and other community liaisons.•Facilitator IV may provide individual psychotherapy, group therapy, and family therapy as reflected in the family's Individual Child and Family Plan.•Facilitator IV will have the opportunity to provide psychotherapy to commercially insured clients for an additional stipend through South Coast's Hope Healing Connection Program. KNOWLEDGE, SKILLS & ABILITIES:•Familiarity with the functionality and phases of Wraparound programs. •Must be proficient in Microsoft Word, Excel Microsoft Outlook, and able to navigate EHR systems. •Can demonstrate knowledge of crisis management skills and can perform such skills as necessary.•Ability to remain organized and manage multiple tasks at once. •Able to establish and maintain appropriate rapport with children, youth, and families.•Knowledge of behavior modification, child development, and/or the child welfare system. •Ability to adopt a Team Concept and work effectively as a member of a multidisciplinary team. •Experience with leading and facilitating groups and meetings.•Knowledge of local community resources for families. RequirementsGeneral Requirements: Facilitator I - Hourly Compensation: $29-$30.60 ($60,320-$63,652.03)Education/Experience1. Bachelor's degree in Social Work, Marriage and Family Therapy, or Psychology.2. At least 2 years of experience working in a human-service related field (Preferably in a program utilizing the Wraparound model). Licensure, Registration, Certification1. Valid CA driver's license and proof of automobile insurance.2. Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.Facilitator II - Hourly Compensation: $30.60-32.20 ($63,653-$66,984.06) Education/Experience1. Bachelor's degree in Social Work, Marriage and Family Therapy, Psychology, or related field.2. At least 2 years of experience working in a human-service related field (Preferably in a program utilizing the Wraparound model).3. Matriculated in a master's program in a related field.Licensure, Registration, Certification1. Valid CA driver's license and proof of automobile insurance.2. Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.Facilitator III - Salary: $66,985-$70,316.09Education/Experience1. Master's degree in Social Work, Marriage and Family Therapy, Psychology, or in related field.2. Experience in human services (preferably Wraparound or similar program utilizing the Wraparound model). Licensure, Registration, Certification 1. Valid CA driver's license and proof of automobile insurance.2. Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.Facilitator IV - Salary: $70,317-$73,648.12Education/Experience1. Master's degree in Social Work, Marriage and Family Therapy, Psychology, or in related field.2. Experience in human services (preferably Wraparound or similar program utilizing the Wraparound model). Licensure, Registration, Certification1. Registered with the Board of Behavioral Sciences (BBS) and obtaining clinical hours. 2. Valid CA driver's license and proof of automobile insurance.3. Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.PHYSICAL REQUIREMENTS:
- Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
- Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
- Required to occasionally lift and carry 10-20 pounds.
- Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
- Regularly required to sit and use computer in office settings.
