Responsibilities:
As an FCT Specialist at Health Connect America (HCA), you will deliver Family Centered Treatment services to children, adolescents, and their families, primarily in their living environments. This role requires FCT certification within one year of employment and involves providing 24/7 on-call coverage to a caseload of families.
Key Responsibilities:
- Service Delivery: Provide FCT services per the service definition and model fidelity requirements. Conduct interventions aligned with the FCT model and facilitate the movement through the four phases of treatment.
- Certification and Training: Complete FCT certification within one year. Participate in internal trainings and FCT Checkoffs through an e-learning system within designated deadlines.
- Clinical Documentation: Complete and upload all required clinical records and FCT documents in the Electronic Health Record (EHR) timely and according to company standards.
- Service Plan Development: Develop, maintain, and implement Service Plans in coordination with the Treatment Team. Conduct ongoing revisions and ensure effective communication and service provision.
- Psychoeducation and Goal Setting: Provide psychoeducation as indicated in the Service Plan. Assist families in setting weekly goals and identifying steps to achieve them.
- Referrals and Discharge Planning: Make referrals to services and resources. Develop and coordinate discharge plans, accessing community resources for aftercare needs.
- Assessment and Coordination: Oversee initial and ongoing assessment activities. Convene the Treatment Team and consult with medical and non-medical providers.
- Behavioral Health Services: Provide and coordinate behavioral health services with other professionals and Treatment Team members. Ensure linkage to appropriate services, including psychological and psychiatric evaluations.
- Quality Assurance: Participate in Quality Assurance/Quality Improvement activities, including audits and improvement plans.
Qualifications:
- Minimum of a Bachelor's Degree in human services or related field is required.
- Two years' experience working with youth or families.
- Current Family Centered Treatment Certification is preferred.
- Additional state specific requirements include:
- NC, VA- QMHP or ability to register as a QMHP
- MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.
