Job Responsibilities:
- Assesses mother and family needs in relationship to the environment.
- Provides assistance in determining the level of mental health/substance abuse care needed by the client and family.
- Administers the functional and other assessment tools outlined in the Program Description Plan.
- Chairs the family treatment and service plan team and develops the final product and subsequent reviews.
- Monitors client progress and assists clients in accessing services from other providers.
- Determines the adequacy and effectiveness of auxiliary services and serves as an advocate when services are deemed inadequate.
- Provides direct services to client and family as treatment needs dictate in the community.
- Arranges for or provides transportation to needed health care and support system appointments.
- Works closely with community agencies and consumers to further develop support services and substance abuse treatment services in the community.
- Coordinates client and family services throughout the agencies involved in the project.
- Provides outreach search to locate missing clients.
- Conducts two home visits per month per client.
- Documents all activities in a timely manner.
- Completes data for program evaluation purposes.
Education: Bachelor's degree in a human service related field required. Must possess or be eligible to obtain a valid WV Social Work License. Experience: Prior experience is required. Must be able to make independent decisions and have strong working knowledge of services available. Must have supportive counseling skills, parenting education skills, and appropriate knowledge of substance abuse and children's issues.
