• Long-Term Relationships: Foster strong, ongoing relationships with patients, becoming a trusted healthcare partner for individuals and families. Focus on continuity of care, ensuring patients receive consistent, personalized treatment throughout their lives.
• Collaborative Practice: Work closely with physicians, specialists, nurses, and other healthcare professionals to deliver integrated care. Participate in team meetings and contribute to the development of care plans that address the unique needs of each patient.

Qualifications:

• Board Certification: Must have current and active Board Certification as a Family Nurse Practitioner.
• NYS License: Must hold a valid and unrestricted New York State (NYS) License to practice as a Family Nurse Practitioner.
• Experience: Prior experience in a primary care setting is preferred, with a demonstrated ability to manage a diverse patient population across the lifespan.
• Clinical Skills: Proficiency in conducting physical exams, ordering and interpreting diagnostic tests, developing treatment plans, and prescribing medications.
• Communication: Excellent interpersonal and communication skills, with a strong ability to connect with patients, families, and healthcare team members.
