🕰️ Job Type: PRN (Minimum 12 hours per week)

⏰ Business Hours:

• Mon-Fri: 8:30 AM - 7:00 PM
• Sat-Sun: 10:00 AM - 6:00 PM
• 🌐 Work Environment: Hybrid-Remote (Field & Home-Based Work)
• 💰 Salary: $80.00 - $90.00/hr. (Weekly Pay)

What You'll Do:

• Conduct Clinical Appointments: Perform clinical evaluations via Telehealth or in-person to assess eligibility for long-term care services, ensuring top-notch care.
• Precept New Clinicians: Train and mentor new clinicians on the Community Health Assessment tool, and clinical best practices.
• Ensure Compliance: Stay up-to-date with regulatory changes to maintain the highest standards for assessments.
• Collaborate & Lead: Work closely with managers to improve processes, enhance care, and lead training initiatives.
• Provide Support: Offer clinical and educational guidance to staff to ensure optimal care delivery.
• Quality Assurance: Engage in quality improvement activities to ensure goals are met.

What We're Looking For:

• A Master's degree in Nurse Practitioner or Physician Assistant program.
• Minimum 2 years of clinical experience as a Board-Certified Nurse Practitioner or Physician Assistant.
• At least 1 year of experience in clinical training or precepting.
• Preferred specialties: Internal Medicine, Family Practice, Adult Health, Psychiatry, or Community Health.
• Experience with adult/geriatric populations, home care, or individuals with disabilities.
• Active New York State Nurse Practitioner or Physician Assistant license.
• Willingness to obtain a New York Medicaid ID Number.
• Must have at least 3,600 hours of NP experience.
• Ability to travel up to 60% within the assigned region.
