  • This is a permanent position in the Post Acute Care setting. *Some Post Acute Care experience is required.
  • 15 -20 patients per day
  • Full-time position
  • Drive high-quality, safe, and cost-effective patient care in post-acute care communities
  • Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Ensure timely documentation and billing through Post Acute specific EMR meeting Medicare guidelines
  • Experienced candidates may be eligible for increased compensation due to prior experience in a post-acute care setting
