Job DetailsJob Location Bronx, NY Position Type Full-Time Education Level MA/MS Salary Range $55,000.00 - $59,000.00 Salary/year Job Shift Monday - Friday 9 AM - 5 PM Description The Assertive Community Treatment (ACT) and Forensic Assertive Community Treatment (FACT) teams provide mobile, community-based mental health treatment to promote recovery and community integration of individuals with serious mental illnesses, including those with co-occurring substance use disorders and criminal justice involvement. The Family Specialist / Employment Specialist is an integral part of a multi-disciplinary team of mental health clinicians providing 80% of treatment services in community settings. These roles require a highly organized, detailed, and assertive individual who is committed to providing person-centered work from a trauma-informed perspective. Essential Position Functions:
- Complete comprehensive assessments and utilize standardized screening instruments for substance use, suicidality, trauma, and violence risk.
- Develop crisis intervention and relapse prevention plans.
- Identify appropriate treatment services and objectives based on consumer needs and treatment plans.
- Address housing, income support, education, vocational training, social support, employment, and primary care needs.
- Collaborate with family members, social supports, and community providers to enhance treatment outcomes.
- Maintain timely and accurate progress notes and reports.
- Provide advocacy and liaison services to secure emergency care and community support.
- Coordinate discharge planning in collaboration with inpatient staff.
- Provide 24-hour crisis intervention on-call services.
- Conduct individual employment and vocational counseling, advocating for consumer access to education and employment opportunities (Employment Specialist).
- Provide 80% of treatment contacts in community settings.
- Complete weekly and monthly AOT (Assisted Outpatient Treatment) reports as required.
QualificationsRequired Knowledge, Skills, and Abilities:
- Masters level in social work mental health counseling or related field, LMSW, LMHC - LP is referred
- At least 2 years of experience providing services to adults with serious mental illness, substance use disorder, criminal justice involvement, and trauma.
- PHYSICAL DEMANDS: While performing duties, the employee is regularly required to talk and hear, occasionally lift up to 20 pounds, and have specific vision abilities.
- TRAVEL: This is a field-based position requiring 80% of services to be provided in the community.

- Fluency in Spanish preferred.
