All staff in positions which regularly require driving either company or personal vehicles while performing work on behalf of Community Action are required to provide Human Resources proof of a valid driver's license for a motor vehicle record check both at hire and annually thereafter. Human Resources will conduct a motor-vehicle record. These records will be held as confidential records in Human Resources. All accidents will be reported to Human Resources within 48 hours.

Position Summary:

In support of the Agency's mission, this position is part of a child abuse prevention program that focuses on promoting caregiver and child bonding, child development, caregiver mental health and wellbeing, and overall family stability by providing introductory and substitute home visiting services to families. This position works closely with diverse family types, including families who identify as LGBTQ+ or other non-traditional family structures, to promote community belonging, social capital, and well-being. This position also supports program efforts in data management and collection, identifying common errors, and recommending changes to workflow as needed. This position works with program staff to ensure that data entry is accurate and complete. Duties include assessment, linkage to services, education, advocacy, evaluation, and developing and generating reports. Duties also include establishing collaborative relationships with service providers and clients while achieving positive results with culturally diverse populations and acting as the program liaison with other internal programs.

Essential Functions and Responsibilities:

  • Work with families, supervisor, and Healthy Families team to promote positive caregiver-child relationships, support healthy growth and development, enhance problem-solving skills, and improve support systems.
  • Engage with families during the pre-enrollment period which includes phone call outreach and conducting Welcome Baby Visits
  • Conduct substitution home visits when a Family Support Specialist is on leave, vacation, or out for any other period of time
  • Effectively work with families and individuals of diverse backgrounds (including teen parents; families with mental health, substance abuse, or domestic violence concerns; families experiencing poverty).
  • Maintain accurate, legible, timely documentation detailing contact and progress of families that guarantees confidentiality.
  • Complete on-going evaluation documentation and assessments for each family in accordance with Healthy Families standards and requirements.
  • Connect families with community resources and support self-advocacy across systems.
  • Encourage and facilitate family participation in program and community activities.
  • Serve as a cohesive member of the team, participate in problem solving and promote innovation.
  • Work in collaboration with Community Action staff and other agencies and partners to coordinate services and resources.
  • Enter data into database(s) as necessary.
  • Maintain current information on community and provide assistance to families and individuals in accessing community resources.
  • Attend staff meetings and trainings as required.
  • Provide parent education, home visitation, and family support services to parents beginning in the prenatal period and frequently extending through preschool age of the child(ren).
  • Implement and administer specified Healthy Families parenting curriculum with families in the program.
  • Strictly adhere to all safety policies and procedures at all times and respond/correct any at-risk safety issues immediately.
  • Strictly adhere to agency code of ethics and standards of workplace behavior.
  • Perform other duties as assigned to meet business needs including regular and reliable attendance and adherence to all company workplace behavior standards.

Required Education/Training/Experience:

  • Minimum of an Associate Degree, Step 7 on the Oregon Registry, CDA, or equivalent with a minimum of one year of case management experience, including maintaining accurate and complete case files and coordination of services with other agencies. An equivalent combination of education and experience may be considered.
  • Ability to work independently and produce quality work under tight timelines.
  • Excellent written and verbal communication skills.
  • Ability to set and maintain positive and professional boundaries with clients.
  • Ability to establish trusting relationships with clients and staff, demonstrating acceptance of individual differences.
  • Willingness to participate in reflective practice (demonstrates a capacity for introspection and an awareness of self in relation to others, recognizes the value of feedback and supervision, etc.).
  • Ability to work effectively and collaboratively with a diverse client and peer population, including experience working with marginalized, vulnerable and/or low-income populations.
  • Intermediate computer literacy and familiarity with applications such as databases and spreadsheets, word processing, email, and internet.
  • Available transportation and proof of auto insurance.

Preferred:

  • Bachelor's Degree in Social Services or a directly related field.
  • Bilingual in English and at least one other language which reflects the current population(s) within Washington County.
  • Knowledge of Washington County social services and resources including services specific to the target population being served
  • Prior Community Action program experience.
  • Registration in Oregon Registry Online with an assigned step

Required Licensing or Other Special Certifications:

  • Requires a valid Oregon driver's license, proof of auto insurance for private vehicle, and access to reliable transportation.

Required Physical Demands of Essential Functions and Responsibilities:

  • Work is performed in normal office environment and/or client's homes.

  • Local driving in various traffic and weather conditions to provider's facilities or homes using personal vehicle.

  • Work is light in nature, lifting maximum of 20 lbs with frequent lifting and/or carrying of objects weighing up to 10 lbs.
