Family Team Facilitator – Juvenile Justice Department

Location:

  • Chicago, IL - South Suburbs
  • Chicago, IL - Southside

Schedule:Monday – Friday (8 hours/day)

Position OverviewThe Family Team Facilitator plays a vital role in supporting youth and families involved in the Juvenile Justice system. Using a wraparound approach, the Facilitator collaborates with families, youth, and their support networks to identify needs and develop personalized care plans.

Key Responsibilities

  • Facilitate meetings with youth, families, and support networks to create care plans.
  • Conduct strength-based assessments to identify needs and resources across all life areas.
  • Build strong relationships with youth, families, caseworkers, and support systems using effective interpersonal skills.
  • Deliver care that is strength-based, family-centered, and culturally responsive.
  • Track service progress and maintain accurate electronic data for quality assurance.
  • Lead family team meetings, complete assessments, and advocate for youth and their families.

Qualifications

  • Education/Experience:
    • Bachelor's degree OR 3–4 years of experience working with high-risk youth in the Juvenile Justice system.
  • Skills and Training:
    • YASI (Youth Assessment Screening Instrument) training preferred.
    • At least 2 years of experience coordinating care and working directly with youth and families.
    • Strong interpersonal and communication skills.
    • Ability to work flexible hours, including evenings and weekends, to accommodate families' schedules.
    • Exceptional attention to detail in service coordination.
