As a Family Youth Partner, you'll be an integral part of the Family Wellness Centers by managing K-8 student and family walk-ins and providing care management, advocacy, education, outreach, mentoring and coaching. As part of the school's Care Team, you will provide parent engagement and outreach of identified at-risk students to support student interventions & family wellness. Family Youth Partners are responsible for assisting clients achieve their wellness goals by providing support services that include: supporting continuity of treatment, groups, transition between programs, and linkages to other community resources. You will be a leader in supporting and developing wellness-driven behavioral health and wellness services and programs for students and families, which may include student clubs, student/parent workshops, and classroom presentations. You'll work collaboratively with dedicated wellness champions, educators and student groups, contributing your knowledge to enhance our students' lives. JOB COMPLEXITY.
- Youth connection: providing 1-on-1 support, sharing health education like coping mechanisms, etc.
- Implementation of youth-driven programs: supporting student clubs with advocacy, identifying gaps in student needs and implementing youth-driven programs
- Facilitating social emotional/life skills/behavioral health groups with students and families
- Parent outreach/engagement and home visits to support student behavior intervention- may involve care navigation and resource referralDevelop individualized client case plans
- Participating in the billing structure for allowable services provided to students and families in the District
- Participating in training required for the Fee Schedule pilot.
- Comply with confidentiality and privacy standards of protected health information and other applicable information and material in accordance to HIPAA and Harmonium policies and procedures
MINIMUM COMPETENCIES.
- Minimum of 2-year degree or combined equivalency of skills and demonstrated experience in the field of social services, human services, social work, counseling, case management, education, or the health services field
- Experience as a consumer and/or family member of someone who was a recipient of any public, community, education, and/or social service agency
- 2 years experience in Case Management, Family Support, Advocacy or related program preferred
- Experience with electronic documentation systems
- Bilingual Spanish HIGHLY preferred
COMPENSATIONBenefits: features medical, dental, vision, life insurance, long-term disability, and critical illness coverage, along with additional voluntary options. We also offer a 403b Retirement Plan with a matching contribution of up to 2%, depending on years of service. Time off accrual for this position includes 8 hours of sick time per month and 10.66 hours of vacation per month for the first three years, increasing to 14 hours per month thereafter.
