Field Registered Nurse (RN) - Home Health Care - Convergence : Job Details

Growing Home Care Agency is seeking a full-time Field RN to join the dynamic and passionate team. This position will play a vital role in providing high-quality and compassionate healthcare services to both clients and their families.

Schedule: Monday - Friday

Full-time and Part-time Available

Responsibilities:

• Conducting initial assessments of new clients to determine their healthcare needs and develop individualized care plans.

• Performing regular home visits to assess client progress and make necessary adjustments to care plans.

• 50/50 patient care & compliance/documentation.

• Collaborating with the interdisciplinary team to ensure proper patient care.

• Providing education and emotional support to clients and their families.

• Maintaining accurate and up-to-date patient records and documentation.

• Participating in team meetings and contribute to the development of care strategies.

Qualifications:

• Current NYS RN license

• One year of nursing experience, preferably in home care or related field

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team

• Compassion and dedication to providing patient-centered care

• Ability to multi task

• Ability to go to the Bronx for home visits

• Reliable transportation and valid driver's license
