Full Time Patient Care Assistant - Day Shift - Mohawk Valley Health System : Job Details

Job Summary

As a Patient Care Assistant, you will work under the direct supervision of a Registered Nurse to provide essential nursing care to non-acutely ill patients. Your role will involve assisting with personal hygiene, daily living activities, and mobility, while ensuring a safe and clean environment. Additionally, you may be responsible for transporting medications as authorized by Pharmacy Services.

Core Job Responsibilities

• Assist nursing staff in delivering basic care to non-acutely ill patients, including personal hygiene and mobility.
• Correctly identify patients before performing any interventions or care.
• Communicate any changes or concerns regarding patients and families to the primary caregiver.
• Observe, document, and report on vital signs, intake and output, and weights.
• Provide personal care such as bathing, oral hygiene, hair care, bed making, ambulation, toileting, and positioning.
• Perform additional duties as necessary to support the nursing team.

Education/Experience Requirements

PREFERRED:

• High School diploma or equivalent.

Licensure/Certification Requirements

REQUIRED:

• Must complete Basic Life Support and behavior management training by the end of the probationary period and maintain certification while in the position.
