Apply today and be part of Sheridan Memorial Hospital's mission of excellence! *JOB SUMMARY* The Restorative Care Program Coordinator is responsible for developing, implementing, and overseeing restorative care programs for residents in a nursing home setting. This role involves working closely with nursing staff, physical therapists, occupational therapists, and other healthcare professionals to ensure that residents receive high quality restorative care aimed at improving or maintaining their physical function and overall well-being. Leads and assists in the development of the care teams and assumes leadership presence in the absence of and as delegated by the Director of Nursing. This includes coordination of the care team schedule and on-call duties on a regular rotation. *ESSENTIAL JOB FUNCTIONS* Key Responsibilities: * * Program Development and Implementation: * Design and implement restorative care programs tailored to the individual needs of residents. * Develop and update restorative care plans based on residents' assessments and progress. * Coordinate with nursing staff and rehabilitation therapists to ensure effective execution of restorative care plans. Resident Care: * * Conduct initial and ongoing assessments of residents' restorative care needs. * Monitor and document residents' progress in restorative care programs. * Provide direct restorative care services as needed. * Works at least one shift weekly as the primary nurse on a rotational basis in each cottage. Staff Training and Support: * * Train nursing staff on restorative care techniques and program implementation. * Serve as a resource for nursing staff, providing guidance and support on restorative care issues. * Ensure that staff follow established restorative care protocols and procedures. * Provides leadership and mentorship to each cottage's care teams. Quality Assurance and Compliance: * * Monitor the quality and effectiveness of restorative care programs. * Ensure compliance with all federal, state, and local regulations related to restorative care. * Maintain accurate and up-to-date documentation of restorative care activities and resident outcomes. Collaboration and Communication: * * Collaborate with the interdisciplinary team to ensure cohesive and comprehensive care for residents. * Communicate regularly with residents' families regarding their loved ones' progress and needs. * Participate in care conferences and other meetings as required. * Regularly visits each Green House Cottage to monitor clinical cares and individual status of the Elders. * Attends self-managed work team meetings as necessary to communicate nursing issues/concerns. Provides input for team discussion and seeks feedback from the team. * Work is performed in a nursing home setting with frequent interaction with residents, families, and staff. Other Duties: * * Stay current with best practices and emerging trends in restorative care. * Participate in continuing education and professional development activities. * Perform other duties as assigned by the Director of Nursing. * May be required to work evenings, weekends, and holidays and on-call. *POSITION QUALIFICATIONS* *Education / Experience / License and Certifications* * * Certificate of Licensed Practical Nurse or Associates degree in Nursing, required * Licensed Practical Nurse (LPN) or Registered Nurse (RN) Wyoming license, required. * Previous experience in management or leadership roles, preferred. * Advanced training or certification in restorative care or rehabilitation, preferred. * Minimum of 2 years of experience in a healthcare setting, preferably in a nursing home or rehabilitation facility, required. * Experience in restorative care or a related field, preferred. * BLS, required. * Central Registry, required. *Additional Skills* * * Strong knowledge of restorative care principles and practices. * Excellent assessment and care planning skills. * Ability to train and support nursing staff. * Strong organizational and time management skills. * Excellent communication and interpersonal skills. * Ability to work effectively as part of an interdisciplinary team. * Proficiency in electronic health records (EHR) systems and other relevant software. * Ability to lift, move, and transport residents as needed. * Ability to stand, walk, and move around the facility for extended periods. * Ability to perform tasks that may involve exposure to blood, bodily fluids, or other potentially infectious materials.
