Minimum Qualifications MINIMUM QUALIFICATIONS:In title Habilitation Specialist 1, Grade 14;OREligible for 70.1 transfer by way of current permanent status in a title deemed comparable to Habilitation Specialist 1 by the Department of Civil Service (must also be within 2 salary grades of grade 14.) ORA bachelor's or higher-level degree in a human services field* * a degree in a human services field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.*QIDP (Qualified Intellectual Disorder Professional) designation requires one year's experience working with people with a developmental disability AND a Bachelor's Degree or higher in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling or psychology).

Duties Description DUTIES:Under the supervision of the Habilitation Specialist 2 the Residential Habilitation Coordinator is responsible for the development, documentation, and implementation of Residential Habilitation Services for people living in state operated community residences. The Residential Habilitation Coordinator will participate with other residential staff to ensure service delivery is focused on Valued Outcomes. The Residential Habilitation Coordinator will also participate in the development and implementation of inclusion plans. May be required to complete investigations as assigned by the Treatment Team Leader. DEVELOPMENT OF A COMPREHENSIVE HABILITATION SERVICE PLAN- A plan is developed for each individual as recommended by their Life Plan, which includes discussions with the individual, the advocate, the Care Coordinator, and the residential provider.- The plan will include the development of a current Safeguard Summary Plan, as well as Personal Expenditure Plan.- The plan will be reviewed monthly and documentation regarding the status will be prepared at least every six months.PROVIDE TRAINING AND TECHNICAL ASSISTANCE TO RESIDENTIAL STAFF- Ensure that staff is trained in the implementation of the residential habilitation plan.- Provide in-service training, topics to include protective oversight, choices, inclusion, personal expenditure plans.- Provide house specific training and assistance to help meet the valued outcomes for each individual.PARTICIPATE IN SURVEY PROCESS- Ensure compliance with agency practices, policies, as well as regulatory policies.- Participate in the Peer review process and other reviews as requested by TTL.- Assist with DQM compliance standards. MAINTAIN THE RESIDENTIAL HABILITATION FILE- Update file according to the approved index.- Complete monthly Residential Habilitation Coordinator notes.- Complete the Personal Information Form. PROVIDE DIRECT SERVICES TO INDIVIDUALS AS NEEDED- Advocate for specific clinical services when appropriate.- Attempt to help individuals meet and develop community ties.- Assist individuals in identifying and pursuing their desires, needs, rights and dreams.CONDUCT CLINICAL MEETINGS- Schedule and coordinate regular meetings of clinical team members at the request of the TTL.- Prepare and distribute meeting minutes.- Facilitate completions of assessments with the team (i.e. money management, bed safety determination, etc.).LinkedIn:

Additional Comments ADDITIONAL COMMENTS:- This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. If you are hired under the HELPS Program, you will be first be appointed as a temporary employee. In May 2024, if you are performing successfully in the position, your appointment will be changed from temporary to permanent non-competitive. Once you are in permanent status, your official probationary period will begin and you may take any promotion exam for which you are qualified.At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status. - Family and household member relationships: OPWDD policy may impact eligibility for this position.
