Job Overview - Health Care Coordinator (RN):Take on a pivotal role with our client as a Registered Nurse Health Care Coordinator in Philadelphia, PA. In this temporary position, you ll coordinate top-quality health care services for individuals with intellectual and developmental disabilities across various locations.Compensation: $55/hourLocation: Philadelphia, PASchedule: Monday to Friday, 9:00 AM to 5:00 PM (Fieldwork Required)Responsibilities as the Health Care Coordinator (RN):
- Health Oversight: Monitor and coordinate comprehensive health services across assigned locations.
- Training: Educate and train staff on relevant medical diagnoses, procedures, and care protocols.
- Community Visits: Conduct home visits to community homes (1-3 residents per home).
- Best Practices: Stay updated on and implement best practices for care delivery for individuals with developmental disabilities.
- Development: Develop and implement personalized health care plans in line with federal, state, and county regulations.
Qualifications for the Health Care Coordinator (RN):
- Education: Bachelor s degree in Nursing (BSN) required.
- Licensure: Active RN license in the state of Pennsylvania required.
- Experience: 3+ years in Nursing required, with experience in care coordination for individuals with developmental disabilities
- Skills: Proficient in Microsoft Office (EHR experience preferred).
- Other Requirements: Valid driver s license, reliable transportation, and successful completion of background and motor vehicle checks.
