Consultant, Community Health Worker

Summary

Our client, a start-up company utilizing healthy food as medicine approach, has engaged GForce Life Sciences to provide a community health worker. This individual will be responsible for supporting and attending local community events to educate potential clients about food benefits available.

Duties / Expectations of Role

  • Build strong relationships with health plan members to educate them about the benefits and eligibility criteria for medically tailored meals. Provide information in a clear and culturally sensitive manner.
  • Engage with community members through various channels, including community events, workshops, and outreach programs. Raise awareness about the importance of medically tailored meals and encourage enrollment.
  • Collaborate with health plan members to assess their eligibility for medically tailored meals. Gather necessary information and documentation to facilitate enrollment in the program.
  • Conduct health assessments in accordance with established protocols. Collect vital health information, identify risk factors, and refer individuals to appropriate healthcare providers as needed.
  • Maintain accurate and up-to-date records of member interactions, eligibility assessments, and health assessment results. Ensure all documentation adheres to privacy and confidentiality regulations.
  • Work closely with interdisciplinary teams, including healthcare providers, social workers, and nutritionists, to ensure seamless coordination of services and resources for health plan members.
  • Build and nurture relationships with local organizations, clinics, and community stakeholders to enhance the reach and impact of the medically tailored meals program.

Mandatory Requirements

· Bilingual with Spanish/English required

· 6+ months of experience in community-level health education or related field required

· Microsoft Office Suite experience required

· Strong attention to detail, and comfortable working in a start-up environment

· Flexible with occasional work after hours and weekends

Nice to Have

· Knowledge of medical insurance billing systems and data analysis

Term & Start

· Benefits Available (Medical, Dental, Vision, 401k)

· Remote - West Coast Based

· Up to 6-month contract to potential perm

· September/October 2024

· Full-time (40 hours/week)

· 20% Travel to Community Events (1-3 days/week)
