The Health Outreach Specialist serves as an extended care team member and provides field-based well checks, and patient membership engagement support services.

Accountabilities Include:

Performs Essential Job Duties

* Coordinates with health care teams to identify patients needing home or field-based services.

* Provides home or field-based services, including environmental assessment, to engage patient members in the coordination of their care.

* Provides virtual or telephonic support services to assist in navigating community resources as needed.

* Advocates for patients and teaches strategies for self-advocacy.

* Communicates with care team, primary care provider, and health plan provider as needed.

* Coordinates with the patient's care team to develop individual patient care plans.

* Ensures the documentation of patient care in EPIC and is responsible for following workflows that result in capturing activities to meet patient engagement standards as outlined in the APCM program.

* -CareSTEP: Accessing Community Resource / Service

* -CareSTEP: Home Visit (non-billable)

* -CareSTEP: Health Education Supportive Counseling

* -CareSTEP: Transportation Assistance

* Participates in approved community events with a focus on patient membership recruitment.

* Coaches patients and their families on the importance of lifestyle changes, adherence to medication regimens, and recommended treatments.

Additional Duties:

* Demonstrates understanding and compliance with all HIPAA regulations.

* Follows all La Clinica policies and procedures and ensures consistent compliance.

* Recommends program specific policies and procedures as needed.

* Performs work in a consistently safe manner, including the utilization of equipment and supplies.

* Complies with all policies and/or guidelines related to employee, patient and visitor safety. (i.e. SMDA, MSDS, OSHA, CLIA).

* Participates as appropriate in safety, disaster and fire prevention programs and drills.

* Identifies and takes initiative to offer suggestions that promote a safe environment.

* Assists leaders to identify the health needs of the community.

* Attends supervisory sessions with team Nurse Case Managers.

* Demonstrates ability to support La Clinica and its sponsors in a proactive manner, both internally and in the community.

* Follows workflows that result in capturing activities to meet patient engagement standards as outlined in the APCM program.

* Completes assigned trainings via La Clinica's learning module system (Relias) by required due date.

* Other duties as assigned.

Qualifications:

Must be able to interact with a variety of individuals at various levels under stressful circumstances while exercising sound judgement, tact, and diplomacy; work productively both independently and in a team setting; communicate with clarity, both verbally and in writing to groups and individuals.

Education and/or Experience:

Required:

* Minimum: High School Diploma

* Achieves Community Health Worker certification requirements, within 6 months of hire and maintains certification throughout employment.

* BLS HealthCare Provider (First Aid/CPR).

* Valid Oregon driver's license, sound running vehicle and maintenance of vehicle insurance required.

* Excellent interpersonal communication and problem-solving skills.

* Skills to intervene and promote reconciliation, compromise, and positive outcomes in difficult interactions.

* Skilled at using electronic health records as applicable to area of work (EPIC, Wisdom).

Preferred:

* Degree in healthcare, social services, or related field

* Medical background

* Basic knowledge of adult learning models

Language Skills:

Must have the ability to communicate with clarity, both verbally and in writing; read, analyze, and interpret complex documents, regulations, policies, and procedure manuals; and create professional business documents. Preferred bilingual English/Spanish.

Other:

* Must have accessible phone service, working computer and/or laptop, and reliable transportation.
