Health Social Care Specialist - FirstHealth of the Carolinas : Job Details

This position will cover Moore, Lee, and Chatham Counties.

Will be conducting a range of activities from administrative tasks to field work. This position will work with assigned patients and help the patient navigate the healthcare system to achieve outcomes, assist the lead in developing care plans with patients; conduct home visits, visit patients in the hospital and conduct follow-up visits by telephone with patient, help advance patient self-management through personal contact and tools, and assist team members in the development of new strategies for health & social care.

Associates degree in public health, community health or comparable degree required, Bachelors degree preferred. Work experience in related field can be accepted in lieu of degree. Valid NC Driver's License with good driving record required.
