Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Work Shifts3:00 P.M - 11:30 P.MJob DescriptionAssignment Level IIIUnder administrative direction, with the widest latitude for the exercise of independent initiative andjudgment, supervises a complex Pharmacy unit or multiple Pharmacy units and/or performs as aspecialty clinical pharmacist in a highly-specialized field within a hospital, clinic or other health caresetting. In addition to performing the duties of Assignment Levels I and II at a more responsible level,the following are typical tasks performed at Assignment Level III:Examples of Typical Tasks:1. Initiates and/or participates in research studies conducted in the specialty area; conducts drugadministration aspects of drug research projects and prepares reports of findings.2. Provides pharmaceutical education to professional staff at lectures, meetings or individually.3. Coordinates the activities of other supervisors in integrating the functions and staff activity ofmultiple Pharmacy units.Minimum QualificationsFor Level I:1. Possession of a valid NY State license to practice pharmacy.For Level II: In addition to meeting the qualification requirements of the lower level:1. Two years of post licensure experience; or2. Completion of a post graduate specialty residency or fellowship program; or3. Current Pharmacy Board Certification.For Level III: In addition to meeting the qualification requirements of the lower levels, an additional:1. One year of post licensure supervisory experience or specialty clinical experience.Department Preferences1. Know the contents of the Hospital Drug Formulary, inclusive of restricted agents and appropriate ordering forms.2. Provides pharmaceutical care in designated practice area(s) to patients from a fundamental to advanced level. Activities include, but are not limited to, participation in patient care rounds, proactive drug therapy reviews and monitoring, providing drug information, patient counseling, management of pharmacist-based dosing protocols. 3. Processes all orders/requests accurately and efficiently, verifying appropriateness of all drugs in reference to the patient's diagnosis, allergies, height and weight. Exercises good judgment in patient care decisions per performance standard and documents patient care activities and patient outcomes as impacted by pharmaceutical care and clinical interventions.4. Submit ADR reports and medication error/good catch forms to the department in a timely manner.5. Contributes to the provision of pharmacoeconomically sound medication therapy by upholding Formulary standards in Emergency Room.6. Promotes a positive work environment and contributes to a dynamic, team focused work unit that actively helps one other to achieve optimal department results. 7. Collaborates with other health care professionals as needed for problem resolution and/or achievement of identified patient specific goals. 8. Participates on Stroke Team, (aid in collection of medication history, providing education, management of blood pressure, reviewing exclusion criteria for recombinant tissue plasminogen activator (rtPA), and facilitating reconstitution and administration of rtPA.9. Serves as a mentor for staff pharmacists I and II supporting clinical and skill advancement.10. Participates in quality assurance and research projects to advance the pharmacy department's goals and the pharmacy profession. Submits research and scholarly work for publication, platform presentations, and/or poster presentations. 11. Seeks opportunities to expand learning beyond baseline competencies with a focus on continual development as a Clinical ED Pharmacy Specialist.12. Presents in-services for pharmacy, medical, nursing, and other staff members in area(s) of responsibility. Develops educational materials and provides educational programs for the pharmacy staff and other hospital personnel.13. Serves as a preceptor for students from the affiliated colleges of pharmacy.14. Know the eligibility requirements of 340B Program.15.
