As a Veterans Housing &Healthcare Case Manager, you will help veterans served by the SSVF team (Supportive Services for Veteran Families) find and secure housing and healthcare.You will meet veteran clients at their homes, in their communities, and at VA hospitals throughout Cook County, connecting them with resources and helping them navigate complex systems such as the VA. This program provides advocacy and financial assistance to help veterans maintain housing, so you will manage those applications, meet deadlines, and build and maintain relationships with landlords and property managers. On the healthcare side, you will advocate for the veterans within the VA system or within community health care services when veterans are not eligible for VA care. This invaluable experience will set you up for a variety of career paths and growth opportunities within our organization.Essential Duties and Responsibilities* Act as a liaison between the SSVF grantee, the VA, community medical clinic, and other healthcare providers.* Coordinate the access of healthcare for Veterans with complex needs and who require assistance accessing health care services or adhering to health care plans* Work closely with the Veteran's assigned multidisciplinary team, including, medical, mental health, behavioral, and case management personnel* Work within SSVF team to provide timely, appropriate, client-centered care in an equitable manner* Collaborate with the healthcare team and Veteran to identify and address system challenges for enhanced care coordination, as needed.Many education and experience paths are eligible for this role:* High school diploma or GED certificate and 5 years of supervised clinical experience* Bachelor's degree in Psychology, Social Work, or related field* Bachelor's degree in an unrelated field and 2 years of supervised clinical experience* Master's degree in Psychology, Social Work, or related fieldTo succeed in this role, you need:* Experience with homeless and low income individuals with mental health or substance use issues* Previous work experience with the Veterans Administration preferred* Strong computer skills required. Must have comprehensive knowledge of Microsoft Office products, including internet and email.
