Job Overview:
The Licensed Practical Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence.
This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse.
Services are furnished in accordance with Program policies.
As a Liaison, interacts with referral sources and potential referral sources to cultivate new relationships and strengthen existing ones.
Primary Responsibilities:
- Understands and adheres to established policies and procedures.
- Implements the nursing care plan for each patient.
- Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
- Initiates preventive and rehabilitative nursing procedures as appropriate.
- Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
- Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Evaluates with the registered nurse the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse.
- Maintains accurate and complete records of observations, treatments, and care of the patient.
- Participates in medical record audits as assigned.
- Attends staff meetings, patient care conferences, and in-services as scheduled.
- Takes on-call duty, nights, weekends, and holidays as assigned.
- Is responsible for:
- Submitting any changes in schedule to the Director of Clinical Services/Nursing Supervisor on a daily basis.
- Participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapists or speech-language pathologists.
- Prepares clinical and progress notes.
- Assists the physician and RN in performing specialized procedures.
- Prepares equipment and materials for treatments.
- Observes aseptic techniques as required.
- Assists the patient in learning appropriate self-care techniques.
Secondary Responsibilities:
- Works collaboratively with referral sources and/or their identified representatives to discuss EverCare service offerings and coordinate referrals to Community Programs.
- Attends community events, networking meetings, and is active within the community to identify potential referral sources.
- Establishes and maintains consistent contact with each potential referral source identified.
- Meets with referral sources and/or their identified representatives to discuss their current needs and inform them of EverCare at Home options that could meet their needs.
- Meets directly with potential patients and acts as a liaison to ensure smooth referral and transition to care when appropriate.
- Provides education regarding the philosophy of EverCare at Home and its various programs and services.
- Represents EverCare at Home at various community events, which may include speaking engagements or educational sessions.
- Presents services accurately, as specified in the Member Handbook, in a clear, concise manner in accordance with company policies and procedures and Department of Health regulations.
- Maintains knowledge and skills through active participation in training, in-services, and conferences.
- Performs other duties and tasks as assigned by the supervisor to ensure optimal department functioning.

Requirements:
- Proof of current LPN license in good standing from the State Of New York
- Minimum of five (5) years' experience in nursing; home care environment preferred
- Acceptance of the philosophy and goals of this Program
- Ability to exercise initiative and independent judgment
