A healthcare network in New York is looking to add a licensed Registered Nurse (RN) to join their team through Rockland & Orange County as an Admissions Nurse. In this role, the Admission Nurse (RN) will be responsible for handling initial visits (two per day) with new hospice patients in their homes. About the Opportunity:

• Schedule: Monday to Friday (rotating to Saturdays)
• Hours: Standard business
• Patient Population: Hospice (two visits per day)

Responsibilities: The Admissions Nurse (RN) will:

• Admit patients to the facility
• Assess and document a patient's medical history, current condition, and vital signs to determine the level of care required
• Verify insurance information, completing admission paperwork, and ensuring all necessary forms are completed accurately
• Perform initial triage to determine the urgency of care needed by patients and allocate resources accordingly
• Educate patients and their families about the facility's policies, procedures, and what to expect during their stay
• Maintain accurate and thorough medical records, documenting all information related to a patient's admission and care
• Act as advocates for their patients, ensuring that their needs and concerns are addressed and communicated to the healthcare team
• Begin the discharge planning process, as needed
• Perform other duties, as needed

Qualifications:

• 2+ years of experience in Home Care, Hospice, and/or Hospital-based Case Management
• Graduate of an accredited School of Nursing
• NYS Registered Nurse (RN)
