Position: Home Care ConciergeHours: 40 hours per week Schedule: 8:00 AM - 4:30 PM | Monday - Friday, with every other weekend rotation. Location: LifeCircles PACE | Holland Join Our Mission to Empower Seniors! At LifeCircles PACE, we believe in creating a supportive and dignified environment for older adults to thrive. As a Home Care Concierge, you'll provide essential, non-clinical care and companionship that make a meaningful difference in the lives of our participants. What You'll Do: Your role will primarily take place in participants' homes or contracted care settings, including Adult Foster Care, Assisted Living, or Skilled Nursing Facilities As a Home Care Concierge, you will:
  • Provide 1:1 supervision and companionship to assigned participants.
  • Facilitate activities and provide gentle redirection when needed.
  • Perform light housekeeping tasks such as washing dishes, sweeping, and tidying up.
  • Prepare meals to ensure participants enjoy nutritious and satisfying food.
  • Accompany participants on bus rides to their homes, the PACE Center, or external appointments.
  • Provide additional support in the Day Center as requested.
What it takes:
  • Experience: At least one year of experience working with frail elderly or long-term care populations.
  • Reliable transportation and a valid Michigan Driver's License.
  • Possess current CPR certification or willing and able to obtain as offered by LifeCircles.
