Job Responsibilities Supports the professional clinical staff by providing direct patient care in the home setting and performing duties as assigned by the Case Manager. Ensures all actions taken in carrying out responsibilities support patient centered care. Required Qualifications

• Minimum of 18 years of age
• Possession of a current valid driver's license and must maintain eligibility for coverage under Mary Greeley Medical Center's insurance.

Preferred Qualifications

• Highschool diploma or GED
• Previous homemaker experience
