Preferred Home Care of New York, a fully licensed Home Care Agency serving the NY Metropolitan area including Nassau, Suffolk and Westchester counties, is seeking an experienced, Home Care Intake Coordinator for our Brooklyn Corporate Office who understands the regulations governing the home care field, related to Medicare, Medicaid and other insurance, and is committed to excellence in patient care. The Intake Coordinator will enjoy working independently and apart of a dynamic and supportive intake team.If you are dedicated to customer satisfaction, highly motivated, and looking to be a part of one of the fastest growing industries this position is for you!POSITION SUMMARYA Home Care Intake Coordinator is responsible for processing referrals received from marketing sources such as hospitals, nursing homes, and community physicians. The Home Care Intake Coordinator will contact each patient to educate them on the services we can provide and assist them with their home care needs for skilled and non-skilled services. In this important position, The Home Care Intake Coordinator will accept the great responsibility of following a patient from the date of discharge through enrollment for long term care, keeping the patient and family members informed at all times to ensure all patients have caring and professional experience with Preferred Home Care.REQUIREMENTS:Two year of experience in home care intakeHigh school graduate; some college credit preferredUnderstanding the requirements for CHHA's and MLTC'sUnderstanding of communicating effectively with employee, patients and their families, medical and community affiliates in order to develop positive relationships.Proficient in Microsoft Office Suite, Constituent Database (HHA Exchange)Investigative ability, highly organized, self-motivated, takes initiative, excellent written and verbal communication and analytical critical thinking skills; able to perform without much supervision.Organized and punctualJOB RESPONSIBILITIES:Receive referrals from referral sourcesVerify patient's insurance to determine eligibilityEducate patients on services we provideSend referral to MLTC for potential enrollment or Medicaid assistanceSend referral to appropriate CHHA for skilled servicesDetermine what services were approved by insuranceEnsure PHC receives authorization for MLTC cases and provide case specifics to PHC HHA Coordination DepartmentBENEFITS: Competitive SalaryPTOSick DaysPaid Legal HolidaysHealth, Dental, and Vision Insurance Offered401(k)Flexible Spending Account (FSA), A Dependent Care Account (DCA) eligibility upon hireWHY WORK AT PREFERRED HOME CARE OF NEW YORK?Preferred Home Care has an amazing, values-based company culture ripe with collaboration, encouragement and camaraderie.
