Home Care Manager - Fox Chase Wellness Center - Home Care : Job Details

We are seeking an enthusiastic, vibrant, and people-oriented Home Care Manager to join our compassionate team. The ideal candidate will have a passion for providing exceptional care, while being a motivating, understanding leader for participants, caregivers and staff. As the Home Care Manager, you will play a vital role in overseeing daily operations, ensuring the highest standard of care, and managing relationships with both participants and caregivers.

Key Responsibilities:

• Manage daily home care operations, ensuring participants receive personalized care that meets their unique needs.

• Cultivate strong positive relationships with participants, families, and caregivers to foster trust and collaboration.

• Lead, train, and motivate a team of caregivers, providing mentorship and support to maintain exceptional standards of care.

• Address concerns or challenges proactively, using empathy and professionalism to resolve issues effectively.

• Conduct regular care assessments and ensure participant satisfaction through consistent follow-ups.

• Create and sustain a vibrant, supportive atmosphere that promotes the well-being of both participants and staff

• Collaborate with family members, healthcare providers, and other stakeholders to coordinate comprehensive care plans.

• Build and foster a respectful and collaborative team culture, that promotes excellence in service delivery.

Qualifications:

• Prior experience in home care, healthcare, or a related field is preferred.

• Exceptional interpersonal and communication skills with an ability to connect with people of all backgrounds.

• Strong problem solving and conflict-resolution skills, with a focus on empathetic de-escalation.

• A genuine passion for helping others, coupled with the ability to inspire and motivate those around you.

• Proven leadership experience with the ability to manage and support a diverse team.

• Excellent organizational skills and attention to detail.

• Resilience under pressure, maintaining a calm and positive demeanor in challenging situations.

• Must be fluent in Spanish.
