UPMC Home Health is hiring a Registered Nurse to join its team in Erie! This position is a Medical/Surgical and Obstetrics/Pediatrics position.

This is a Full-Time position working daylight from 8 AM- 4:30 PM. On-call shifts and rotating weekends are required.

Responsibilities:

  • Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Assigns other nursing personnel, including LPNs and Aides to assist in the appropriate delivery of services as ordered on the plan of care to the patient.
  • Supervises and teaches other nursing personnel including LPNs and Home Health Aides. Identifies, investigates, and recommends solutions to organizational issues.
  • Identifies any deficits or exceptional performance issues of other nursing personnel, LPNs, and Aides, and recommends solutions. Proven ability to facilitate change.
  • Coordinates case management activities within a multi-disciplinary health care team to ensure appropriate delivery of services to patient.
  • Initiates, coordinates, and ensures ongoing communication with the physician and other members of the health care team.
  • Attends and participates in patient care conferences, team meetings, and other related meetings.
  • Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC Home Health Care towards patients, visitors, staff, peers, physicians, and other departments within the organization.
  • Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of the services we provide.
  • Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care.
  • Initiates, coordinates, and evaluates patient and family-centered health teaching.
  • Establishes the plan of care in collaboration with the physician, patient, and UM Professional by the physician's orders and patient needs.
  • Advises and consults with family and other agency personnel, as appropriate.
  • Completes documentation including visit assessment, OASIS, 485, and Verbal Orders accurately and promptly on the laptop device or in written documentation while in the patient's home and within the agencies and regulatory standards. Data syncs (transmits) information same day. Exercise independent judgment in matters concerning emergent and non-emergent patient care needs and communicate with the physician
  • Participates in on-call coverage rotation for the agency
  • Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patients.

  • Completes an initial and comprehensive assessment of the patient.
  • Constructively receives feedback and direction.

  • Initiates, coordinates, and ensures ongoing communication with the physician and other members of the interdisciplinary healthcare team, including evaluating patients and family-centered health teaching.
  • Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care. Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members by the physician's orders and patient needs.
  • Assign other interdisciplinary team members as appropriate to assist in the appropriate delivery of services as ordered on the plan of care to the patient.
  • Supervises and teaches other nursing personnel including LPNs and Home Health Aides as needed.
  • Attends and participates in case conferences, team meetings, and other work-related meetings.
  • Recognizes limitations and requests assistance and direction to meet performance goals.
  • Completes all necessary documentation which may include visit assessment, plan of treatment, OASIS, 485, and Verbal Orders accurately and promptly in the electronic documentation system while in the patient's home and within the agencies and regulatory standards. Data syncs (transmits) information same day.
  • Identifies, investigates, and recommends solutions to organizational issues.
  • Exercises independent judgment in matters concerning emergent and non-emergent patient care needs and communicates with the physician.

  • Participates in weekend and on-call coverage rotation for the agency as per BU guidelines
  • .Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, including complex and/or specialized care.
  • Zero (0) to two (2) years of experience preferred.
  • Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
  • BSN preferred.
  • Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
  • Ability to work productively and effectively within a complex environment and handle multiple/changing priorities and specialized equipment.
  • Good clinical judgment with critical thinking, analytical, and problem-solving abilities are required as related to various aspects of patient care.
  • Critical thinking skills are necessary to exercise and to lead others in the application of the nursing process.
  • Mobility and visual manual dexterity.
  • Physical stamina for frequent walking, standing, lifting, and positioning of patients.
  • Successful completion of Home Health/Hospice Orientation upon hireLicensure, Certifications, and Clearances:
  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
  • CPR is required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
  • Graduate nurses must complete the licensure examination within SIX (6) MONTHS of hire or one year of graduation, whichever comes first. Though temporary permits are valid for one year from the date of graduation, not one year from the issue of the permit (or until the results of the examination are known at which time it becomes null and void), UPMC requires that GNs take the licensure examination within 6 months of their start date or they will be terminated or demoted from the GN position, at the discretion of the BU.

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
