Department: 39109 Hospice North Region - Hospice Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: This is a Mon-Friday job with patient visits scheduled between 8am and 5pm. Requires travel. Oncall weekend and holiday shifts rotation 8am to 5pm (1 weekend every 3-4 weeks; 1 summer holiday; 1 winter holiday). Assignment will be in Niles, Morton Grove, Glenview, Skokie and neighboring Chicago areas. Major Responsibilities:
  • Contributes to effective patient outcomes. Responsible to provide competent patient care assigned.
    • 1)Demonstrates competent home health aide skills.
    • 2)Promotes patients to reach functional status goals by providing assistance with ADL's following the HHA assignment record in rendering patient care and maintaining safety.
    • 3)Notes changes in patient's condition requiring further attention, informs the case manager, or the RN/Supervisor and documents accordingly.
    • 4)Understands the hospice philosophy, comfortable providing specialized care to the terminally ill.
    • 5)Performs personal care needs, feeding patient, laundry, light cleaning of patient room and washing.
    • 6)Meeting safety needs of patient using equipment safely and properly.
    • 7)Taking temperatures, pulse, respiration and blood pressure as directed by the home health aide assignment sheet when assigned.
    • 8)Provides skin care and changing patient position to prevent breakdown as assigned.
    • 9)Changing patient bed linens and clothing as requested by the patient and family.
  • Responsible for conducting general work activities consistently
    • 1)Exhibits the ability to work independently.
    • 2)Attends to personal work schedule in a consistent and professional manner.
    • 3)Demonstrates ability to cooperate and work well with team members.
    • 4)Secure with issues of death and dying. Provides a calm manner when in a patient's home.
  • Demonstrates effective communication skills.
    • 1)Utilizes appropriate communication skills and effective interpersonal skills with all customers.
    • 2)Actively participates in achieving patient outcomes through case conferencing and IDG's.
    • 3)Prepares documentation reflecting patient's care; adheres to documentation as required by agency, state, federal, and Accrediting Organization standards with time frames.
    • 4)Maintains patient confidentiality, observing of HIPPA/ Privacy acts.
  • Participates actively in CQI Process.
    • 1)Demonstrates cost-effective measures in patient care.
    • 2)Assists with problem identification and solutions.
    • 3)Participates in quality, improvement projects when assigned.
    • 4)Demonstrates measures to ensure patient satisfaction.
  • Adheres to regulatory requirements.
    • 1)Maintains annual inservice hours, Hospice education hours and OSHA requirements.
    • 2)Cooperates in scheduling of required supervisory visits.
    • 3)Provides home health aide services in accordance with all regulatory rules and standards.
    • 4)Meets the training requirements of State and HIPPA.
  • Demonstrates professionalism and works in a cooperative manner with all associates.
    • 1)Demonstrates flexibility in scheduling to support department and other service offices when needed.
    • 2)Identifies own developmental needs and contributes to learning plan.
    • 3)Demonstrates self-direction for maintaining home health aide skills.
    • 4)Demonstrates commitment and supports team, office, company and organizational goals.
Education/Experience Required:
  • High school education or equivalent. One year nurse's aide experience. RN nursing students that meet state requirements.
Knowledge, Skills & Abilities Required:
  • Recognize the needs and behaviors of a variety of age groups of patients treated, understand child growth and development, i.e. understand adolescent behavior and look for signs of normal aging. Written and verbal competency of the English language.
  • Must be certified as a nursing assistant in the State of Illinois
Physicial Requirements and Working Conditions:
  • Must be able to travel to all of the service office boundaries in all extremes of weather on all types of road including expressways. Must be able to assist with ambulation including turning and transferring patients up to 200 pounds. Required to make visits to private residences which may not be accessible to people with disabilities.
  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
