Home Health Aide - Grand View Hospital : Job Details

Under the direction of the professional nurse, the Home Health Aide assists the patient in the home setting with personal care activities and incidental duties which the patient requires. The Home Health Aide assures the assessment of care is appropriate to the age of patients served: adolescents, adults and geriatrics.

Essential Functions:

• The Home Health Aide provides personal care to adolescent, adult and geriatric patients
• Assessing the patients temperature, pulse, respirations, and blood pressure to monitor his/her condition.
• Changing and making bed to maintain cleanliness
• Assisting patients with prescribed exercises
• Assisting patient with application of TED/JOBST stockings.
• Observing patients condition and reporting changes in condition to the Registered Nurse or Supervisor.
• Assisting with patient transfers, ambulation, and prescribed exercises according to established procedures.
• Documentation of care provided and other pertinent observations

Grand View Health's Home Care department serves Bucks and Montgomery communities. Grand View Health Home Care strives for excellence by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital. Gas mileage will be reimbursed.

Schedule: Per diem, between the hours of 8:00 am - 4:30 pm

Education and Experience:High School Diploma/GED, required

Certified Nursing Assistant (C.N.A) or must have successful completion of formal training program with required 75 hours of training and 16 hours of supervised practical or clinical training

License and Certifications:

Basic Life Support (BLS), upon hire, requiredDrivers License PA, upon hire, requiredAutomobile insurance/registration, upon hire, required
