OverviewHome Health Aides (HHA) provide personal care and related services in the home, under the instruction of the RN Case Manager. Tasks to be performed by an HHA must be assigned by the plan of care and performed under an RN who will be responsible for the client care/services provided by the HHA. Under no circumstances may an HHA be assigned to titrate or change the settings of oxygen concentrators, receive or reduce any intravenous procedures, or any other sterile or invasive procedures, other thanrectal temperatures or enemas.
RESPONSIBILITIES
Follows the plan of care to provide, safe, competent care/service to the client Helps the client to maintain good personal hygiene and assists in maintaining a healthy, safe environmentPlans and prepares nutritious mealsAssists the client with ambulation as approvedEncourages the client to become as independent as possible according to the care plan Attempts to promote client's mental alertness through involvement in activities of interestGives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmit trust and confidentialityObserves and reports any change in the client's mental or physical condition or in the home situation to the supervisorPerforms routine housekeeping tasks as related to a safe and comfortable environment for the clientPrepares a progress note promptlyConfirms, on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinatedWorks with personnel of other community agencies involved in the client's careAttends in-service as required by State regulationConfers with the RN Case Manager about the clients status and carePromptly report any change in client condition to the RN Case ManagerWorks within the Home Aides scope of practiceJOB CONDITIONS / REQUIREMENTS
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.On occasion, may be required to bend, stoop, reach and move client weigh; lift and/or carry up to 30 pounds.Must be able to effectively communicate in many methods including verbal, written, and visual.Must adhere to company guidelines and policies as set forth in the contractEQUIPMENT OPERATION
